Notes
This is a four-pin device and replaces the original six-pin device.
Pins 1 and 2 of the new four-pin device are the same as the original six-pin IC.  Pins 3 and 4 are the
equivalent of pins 4 and 5.  See picture below for more detail:

This requires that the new opto-coupler is fitted diagonally, so that the pins are correctly aligned.
