A.  Change of sub transformer (P/N: T102). 
 
The (sub) transformer was changed to new type, CLT5I002ZE for improvement of efficiency.   
 
The new (sub) transformer type, CLT5I002ZE is replacement of previously used (sub) transformer 
 type, 
CLT(KLT)5J033YE, and limited tests were performed for changed new (sub) transformer. 
 
B.  Change of applicant, manufacturer and factory address for listed name “Anam Electronics Co., Ltd”, 
 
due to revision of administrative districts;
